Medical Grounds Application

Any child with a serious medical reason for needing to attend a particular school can apply on medical grounds.  For primary age children, a serious and continuing medical condition of a parent which would prevent them taking their child to school may also be relevant.

Supporting evidence should set out particular reasons why the academy is the most suitable and the difficulties that would be caused if the child had to attend another school. The medical reason must be verified by a GP or consultant, and both the completed medical form and the supporting statement from the GP or consultant must be submitted with the application.  This must be returned directly to Atwood Primary Academy.

Claims for priority of admission on medical grounds will not be considered if submitted after a decision on the original application has already be submitted. Decisions on priority of admission on medical grounds will be made by the Governing Body. “Parent” is defined as the father, mother, foster father, foster mother, legal guardian, or the person who has full parental responsibility for the child.

How to Apply

Please complete the Medical Grounds Form and return to Atwood Primary Academy along with all supporting documentation.  Your case will be heard by a panel and you will be notified of the decision by letter.
